Annovera Replacement Policy
If your Annovera ring gets lost or damaged, the manufacturer, VitaCare, will send you a free replacement and Simple Health will help facilitate the process.
Here's how the replacement process works:
1. Fill out the Patient Information sections of the Annovera Replacement Request Form
2. Email us the pdf at firstname.lastname@example.org with the subject line 'Annovera Replacement"
3. We will fill in the Prescriber Information section and submit it directly to VitaCare at email@example.com
4. Upon receipt and approval of the form, vitaCare will ship a replacement ring directly to you.
A few criteria apply:
- The ring was lost or damaged within 12 months of receiving of it
- You have not previously received a replacement of Annovera. Every patient is limited to one Annovera replacement within their lifetime regardless of many they received or used in the past
- the Replacement Authorization Request is completed and submitted within 30 days of the loss or damage.
- VitaCare reserves the right to amend or end this program at any time without notice.
