Annovera Replacement Policy

If your Annovera ring gets lost or damaged, the manufacturer, VitaCare, will send you a free replacement and Simple Health will help facilitate the process. 

Here's how the replacement process works:

1.  Fill out the Patient Information sections of the Annovera Replacement Form

2.  Email us the pdf at support@simplehealth.com with the subject line 'Annovera Replacement" and we will fill in the Prescriber Information section

3. We will return the completed form to you and you will submit it directly to VitaCare at customerservice@vitacarerx.com

4. Upon receipt and approval of the form, vitaCare will ship a replacement ring directly to you. 

A few criteria apply:

- The ring was lost or damaged within 12 months of receiving of it

- You have not previously received a replacement of Annovera. Every patient is limited to one Annovera replacement within their lifetime regardless of many they received or used in the past

- the Replacement Authorization Request is completed and submitted within 30 days of the loss or damage.

- VitaCare reserves the right to amend or end this program at any time without notice.
